Physical Therapist - Home Health

Location: Greensboro, North Carolina
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Description

Graduate degree from an accredited Physical Therapy Program (approved by the APTA)
Minimum of 1 year physical therapy experience; home health or facility experience preferred
Current and unrestricted Physical Therapy licensure required
Current CPR certification required
Position will be based out of the High Point location

Required Qualifications

Valid Physical Therapist license in North Carolina
Minimum of 1 year of physical therapy experience within the last year
BLS (Basic Life Support) certification

Compliance and Documentation Requirements

Government issued photo ID required
7-year background check within 1 year of start, updated annually
Annual EPLS/GSA/SAM and HHS/OIG checks
Primary source verification of state license
Medication test/pharmacology competency exam as applicable
Competency exam as applicable by specialty
Physical exam within 1 year of start
Negative TB test or chest X-ray per protocol
Required immunizations or proof of immunity: Measles, Mumps, Rubella, Hepatitis B, Varicella
Negative 9-panel drug screen within 1 year of start
Flu vaccination or declination required during flu season (Oct 1 - Mar 31)
Two professional references within 3 years of start

ACLS (Advanced Cardiac Life Support)
BLS (Basic Life Support) - Required
PALS (Pediatric Advanced Life Support)
NRP (Neonatal Resuscitation Procedure)

Additional Information

Candidate will be fitted with an N95 mask onsite; mask fit records maintained by employer
