Job Summary
Seeking a Licensed Practical Nurse (LPN) for a Long Term Care / Transitional Care setting. The position involves working night shifts on weekends with a floating requirement between units.
Schedule & Shift Details
- Shift: 3 x 8-hour night shifts (Friday - Sunday)
- Weekend requirement: Every weekend (Friday through Sunday)
- Floating: Yes, typically 3 shifts in Transitional Care Unit (TCU) and remaining shifts floating to other areas
- Locals accepted within a 50-mile radius
- Pending license accepted
- No on-call required
Requirements/Qualifications
- Valid LPN/LVN license in Washington
- Minimum 2 years of Long Term Care experience, including experience within the last year
- Experience in Long Term Acute Care and Post Acute settings
- Completion of at least one travel assignment
- BLS certification issued by the American Heart Association
- Experience with central line blood draws
- Experience starting IVs
Required Certifications & Licenses
- IV Certification (as applicable)
- Other certifications may be required based on clinical track and specialty
Additional Requirements
- Reliable transportation required (travelers must have their own transportation)
- Must be able to accommodate block scheduling
- Facility may call off traveler up to 1 shift per pay period (every 2 weeks)

Compliance & Documentation Requirements
- Physical exam within 1 year of start, signed by MD, NP, PA, or DO
- Immunizations and health screenings including:
- TB test or chest X-ray (annual update required)
- Tetanus/Diphtheria/Tdap (within 10 years)
- MMR (Measles, Mumps, Rubella) vaccination or positive titer
- Hepatitis B vaccination or signed declination
- Varicella vaccination or positive titer
- Flu vaccine or declination during flu season
- COVID vaccine or declination
- 10-panel drug screen within 90 days of start (no instant tests)
- Respirator fit testing (annual, travel only)
- Background checks including state-specific searches and education verification
- References: Minimum 2 supervisor references covering 1 year of employment in a comparable specialty/setting within the last 3 years
- Employment application covering at least last 5 years of employment
Training & Competency
- Medication and pharmacology competency exams as applicable
- Competency exams required by specialty
- Facility-specific and regional mandatory training modules to be completed prior to start and annually thereafter
- Epic system training may be required depending on assignment
- Other specialty-specific certifications and training as required by clinical track
Notes
- Candidates must comply with floating policy and may be required to float to other units within reasonable distance
- Locals within 50 miles are accepted at the same rate as travelers
- All certifications and background checks must be current and compliant with state and facility requirements
