Job Opportunity: Physical Therapist - Outpatient

Location: Florence, Oregon
Employment Type: Travel/Contract
Contract Length: 13 weeks

Position Overview

• Specialty: Therapy/Rehabilitation (Physical Therapist)
• Shift: Day position, 4 days a week, 10-hour shifts (4x10s)
• Start Date: ASAP (typically 4 weeks from offer for compliance)
• Work Schedule: Monday - Friday preferred; block schedules possible (Mon-Th or Tue-Fri)
• Guaranteed Hours: 40 hours per week
• Patient Load: 9 - 11 patients per day (depending on schedule and census)
• Productivity Standards:

  • Outpatient: 0.45 and better
  • Split between Inpatient/Outpatient: 0.6 and better

Experience & Licensure Requirements

• Years of Experience: Minimum 1 year experience preferred
• Additional Experience: 1 year of Epic electronic health record experience preferred
• Licensure: Oregon Physical Therapist License required (pending state license accepted)
• Certifications:

  • Basic Life Support (BLS) required
• First-Timers Accepted: Yes
• Prior Travel Experience: Not specified as required

Job Duties & Responsibilities

• Treat orthopedic general cases including total joints, acute/chronic pain, decreased balance, general neurology, general medical conditions, upper extremity injuries, and splinting
• No on-call required
• No floating within scope to meet facility needs; may cover as Admission Coordinator/Rehab PPS Coordinator if needed

Additional Requirements & Compliance

• Background checks required prior to start (including SSN trace, criminal, federal, and state-specific checks)
• Physical exam required within 1 year of start
• Fit mask test required within 1 year and updated annually for onsite candidates
• Immunizations and health documentation required:

  • Tdap vaccine or declination
  • Negative TB test or chest X-ray within 1 year
  • Measles, Mumps, Rubella (MMR) immunity or declination
  • Hepatitis B immunity or declination
  • Varicella immunity or declination
  • Drug testing within 30 days of start
  • Flu vaccine or declination during flu season (Nov 1 - Mar 31)
• Orientation modules and quizzes must be completed with 100% score prior to start
• Compliance with facility-specific policies including dress code, masking, and personnel restrictions due to illness

Notes

• Locals are not accepted for this travel position
• No weekend shifts required (Monday - Friday preferred)
• No floating to sister facilities except for specific coordinator roles
• Candidate must review and comply with facility orientation materials and policies prior to start

This position is suitable for Physical Therapists with at least 1 year of experience who hold an Oregon PT license and are comfortable working in an outpatient critical access setting with a focus on orthopedic and neurological rehabilitation.
