Location: Baraboo, Wisconsin
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Description

• Position: Hospice House LPN/LVN
• Setting: Four-bed hospice house caring for patients with a terminal diagnosis, typically with four weeks or less to live
• Experience: Minimum 2 years required
• Schedule: Every other weekend
• Certifications: BLS required
• System Experience: Epic experience required
• Note: First-time travelers are not accepted

Required Credentials & Compliance

Licensure & Verification

• Valid Wisconsin LPN/LVN license or compact license
• Primary source verification required prior to start
• NURSYS verification required prior to start

Certifications

• Basic Life Support (BLS)
• Advanced Cardiac Life Support (ACLS) - check unit requirements
• Pediatric Advanced Life Support (PALS) - if applicable
• Neonatal Resuscitation Procedure (NRP) - if applicable

Background Checks

• Criminal background check within 12 months of start, including:

  • SSN Trace
  • 7-year county residence/work history
  • Nationwide search
  • Sex offender search
• State-specific caregiver screening required for Wisconsin prior to start

Health & Immunizations

• Negative TB test (PPD, Qgold, or TSPOT) within 12 months; chest x-ray if positive PPD within 5 years
• TB questionnaire at start and annually
• Flu seasonal vaccination or approved medical/religious exemption (renewed annually)
• COVID vaccination preferred (initial series)
• Hepatitis B titer, series, or declination (required for new candidates)
• Rubeola (Measles), Mumps, Rubella immunity or vaccination proof (Aya standard for new candidates)
• Varicella immunity or vaccination proof (Aya standard for new candidates)
• Negative drug screen within 12 months of start

Physical & Fit Testing

• Physical exam within 1 year indicating good health (Aya standard for new candidates)
• Respirator fit test completed onsite during first week of orientation; must submit fit test record within two weeks of start
• OSHA Respirator Clearance required prior to start (not required for some non-patient facing roles)

Orientation & Training Requirements

• Completion of all required facility-specific modules annually via LMS platform
• Review and sign:

  • Confidentiality and Security Agreement
  • Standards of Ethical Conduct and Relevant Policies
• Post-start orientation checklists required for Home Health and Hospice candidates
• Glucometer assessment during orientation (if applicable)

• Government-issued photo ID (Driver’s License or State ID)
• I9 verification and E-Verify compliance
• Residency declaration form if working on a compact license in a different state
