Physical Therapist - Acute Care Hospital (Travel/Contract)

Location: Middlesboro, Kentucky
Shift: Day shift, Monday - Friday, 8:30 AM – 5:00 PM (40 hours/week)
Contract Length: 13 weeks

Job Description

• Provide physical therapy services in an inpatient setting.
• Morning shift, Monday through Friday.
• Open to first-time travel therapists.

Required Qualifications

• Valid Physical Therapist license in Kentucky.
• Minimum of 1 year of Physical Therapist experience, including experience within the last year.
• Experience working in an inpatient setting.
• Basic Life Support (BLS) certification issued by the American Heart Association.
• Valid Driver's License.

Compliance & Documentation Requirements

• Valid, unexpired government-issued photo ID (e.g., driver’s license or state ID).
• Background check per facility’s specific bundle including 9-panel drug screen (no THC) with expanded opiates and oxycodone; must be completed within 30 days prior to start.
• Drug testing must be completed through specified testing protocols within 30 days of start.
• Physical exam or physician statement within 1 year prior to start, confirming clearance to work without restrictions.
• Immunizations and health screenings required prior to start:

  • Negative TB test (Qgold only) within 1 year, renewed annually.
  • Proof of MMR (Measles, Mumps, Rubella) vaccinations or positive titers.
  • Hepatitis B series, titer, or declination within 1 year.
  • Varicella vaccination or positive titer.
  • Tetanus/Diphtheria/Tdap vaccination valid within 10 years (Td not accepted).
  • Seasonal flu vaccination or approved medical/religious exemption during flu season.
• Fit mask test to be completed onsite; PAPRs not accommodated if facial hair present. Medical or religious exemptions must be submitted on day one.
• Must complete facility-specific employee health attestation and upload required health documentation screenshots (TB results, OSHA respirator medical evaluation, flu vaccine/exemption, TB risk assessment).

Orientation & Onboarding

• Orientation sessions held bi-weekly.
• Start date on job posting is the required start date.
• Assignment confirmation must be signed within 24 hours of offer acceptance.
• Resource must submit all required documentation promptly; no edits to return-to-office (RTO) requests after offer acceptance.

Additional Requirements

• Two professional references within 1 year of start date required; one must be from a supervisor or higher. Charge RN acceptable for one reference if applicable.
• Complete employment application/resume with education and work history.
• Seven-year work history verification required, preferably through a third-party vendor; attestation letters accepted if verification attempts are exhausted.
• Annual renewal of skills checklist and competency exams as applicable.

Notes

• Prior travel experience is not required; first-time travelers are welcome.
• Candidates must be willing to comply with all facility-specific policies and documentation requirements.
• Locals within 75 miles of the facility are not eligible for this travel assignment.
