Job Opportunity: Home Health Registered Nurse

Location: Seaside, Oregon
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Details

• Shift: Day shift, 8 hours per day (8:00 AM - 4:30 PM), 5 days a week (40 hours/week)
• Schedule: 8-hour days only; no 4x10 or 3x12 shifts available
• Patient Ratio: 1:1; may visit up to 6 patients per day (30 productivity points standard)
• Weekend Work: Occasional weekend coverage may be requested for sick calls but is not common
• Floating: No floating between units required
• Block Scheduling: Not accommodated
• Locals: Candidates within 50-75 miles considered local and accepted
• Transportation: Traveler must have their own transportation; daily travel radius up to approximately 60 miles (60-90 minutes total drive time)
• Electronic Health Record: EPIC required (remote client and mobile app use)

Required Qualifications

• Valid Registered Nurse (RN) license in Oregon
• Minimum 1 year of experience in Home Health nursing within the last year
• Basic Life Support (BLS) certification issued by the American Heart Association or Canadian Heart and Stroke Foundation
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Experience working in a Home Health setting

• Type of Facility: Home Health
• Attire: Business casual; scrubs allowed but not required; no jeans
• Charting: EPIC Remote Client (laptop), Rover app (phone), and OASIS documentation
• Parking: Free parking available
• Mileage Reimbursement: IRS standard mileage reimbursement

Compliance & Documentation Requirements

• Background Check: Must be completed within 90 days of start, covering all locations lived/worked in past 7 years; includes SSN trace, death master index, nationwide checks, motor vehicle record (required for home health), and state-specific abuse/neglect searches as applicable
• Drug Screen: 9-panel drug screen within 12 months of start; instant tests acceptable
• Physical Exam: Within 1 year of start, signed by MD, NP, PA, or DO, indicating fitness for duty and free of infectious disease
• Immunizations & Health:

  • Tetanus/Diphtheria/Tdap within 10 years (declination accepted)
  • Negative TB test or chest X-ray within 12 months (annual updates required)
  • MMR (Measles, Mumps, Rubella) vaccination or positive titer with acceptable documentation
  • Hepatitis B vaccination or signed declination
  • Varicella vaccination or positive titer
  • Seasonal flu vaccine during flu season (declination accepted)
• Respiratory Safety Clearance: Required annually for travel candidates; includes respirator medical clearance questionnaire
• Fit Mask Testing: Required annually for travel candidates; onsite testing available if needed
• References: Minimum 2 references covering 1 year of employment within last 3 years in a comparable specialty/setting
• Employment Application: Must include at least last 5 years of employment with month/year format for dates
• Skills Checklist: Annual update required for applicable units

Additional Notes

• Guaranteed Hours: Facility may call off up to one shift per pay period (every two weeks)
• On-Call: Not required
• Block Scheduling: Not available
• Pending License: Accepted
• Return to Office Restrictions: Confirm with hiring manager
• Background Check Exceptions: Returning travelers with less than 30-day gap may not require new background check unless expiring on assignment
• Annual Regulatory Training: Not required for home health at this facility
