Physical Therapist - Outpatient (Travel/Contract)

Location: Wichita Falls, Texas
Contract Length: 13 weeks
Shift: Day shift, 4 days per week, 10 hours per day (8:00 AM – 6:30 PM)
Schedule Note: May consider 5x8s; preferred start date within the next 3-6 weeks

Job Description

• Outpatient physical therapy role on a travel/contract basis.
• Full-time, 40 hours per week.

Required Qualifications

• Valid Physical Therapist license in Texas.

Experience

• Specialty: Therapy/Rehabilitation (Physical Therapy).

  • Mumps: 2 vaccines or positive titer (exempt if born before 1957)
  • Rubella: 2 vaccines or positive titer (exempt if born before 1957)
  • Hepatitis B: Positive titer, series of 3, or declination
  • Varicella: 2 vaccines, positive titer, or history of chickenpox
  • Seasonal Flu vaccine required during flu season (10/1–5/31); medical or religious exemptions subject to facility review
  • COVID-19 vaccination documentation required
• Drug testing: Negative 9-panel drug screen within 1 year of start; plan for completion within 72 hours of signing contract.
• References: 2 references required within 3 years of start.
• Employment application or resume required.
• Skills checklist required and must be updated annually.
• Orientation and facility-specific documents must be completed prior to start, including safety manual acknowledgment and various compliance forms.
