Overview

• Travel/Contract assignment in Cedar Rapids, Iowa
• Inpatient unit support
• 13-week contract
• Day shift: 4x10 hours, 7:00–17:30
• Schedule options mentioned: Tues–Sat (5x8s) or Wed–Sat (4x10s)

Required Qualifications

• Valid Occupational Therapist license in Iowa
• Occupational Therapist (1 year of experience)
• Occupational Therapist experience within the last year
• BLS - Basic Life Support issued by American Heart Association
• Experience with Epic
• 2 supervisory references from the specialty the worker will be working in
• 7 years of employment history, or back to college/technical school graduation, with no gap greater than 30 days
• Unit-specific skills checklist
• Work history verification for the last 7 years of employment
• 10-panel drug screen including THC within 30 days of start
• Negative TB or chest X-ray within 90 days of start
• Annual physical or statement of health within 1 year of assignment start
• Fit mask test within 12 months
• Tetanus/Diphtheria/Tdap within 10 years or Aya declination
• Hepatitis B series, positive IgG titer, or declination on file
• Varicella immunity or 2 live vaccines
• Rubeola (Measles) immunity or 2 live MMR vaccines
• Mumps immunity or 2 live MMR vaccines
• Rubella immunity or 2 live MMR vaccines
• Flu vaccination or approved exemption status on file by the seasonal deadline
• Iowa Mandatory Reporter Certification - Child Abuse Training
• Iowa Mandatory Reporter Certification - Dependent Adult Abuse training
• Education verification for highest level of education
• Transcript, diploma, or third-party verification for degree
• Background check within 30 days of start
• OFAC / sanctions screening within 30 days of start
• Sex offender search within 30 days of start
• EPLS/GSA/SAM screening within 30 days of start
• HHS/OIG screening within 30 days of start
• Medication test/pharmacology competency exam with an 80% pass rate
• Color blind testing with a 100% passing score
• Competency exam for each department the traveler is assigned to or floating to
• Core Part I, Core Part II, and Core Part III with an 80% score or better
• Physical exam signed by MD, DO, NP, ARNP, or PA
• NURSYS verification for RN licensure when applicable

Preferred

• NIH Stroke Certification may be required
• Quantitative lab results are preferred for immunity titers, though qualitative results are accepted

• The assignment is for OT support on inpatient units
