Job Description

• Acute Inpatient Case Manager role in a large hospital setting
• Manage a caseload of approximately 25 patients
• Responsible for facilitating progression of care, evaluating for the next appropriate level of care, and discharge planning
• Must have a minimum of 2 years of experience in a large hospital setting

Required Credentials & Certifications

• BLS (American Heart Association)
• Current nursing license with Nursys verification within 30 days prior to start
• License/Certificate verification within 30 days prior to start
• Ascension-specific onboarding and competency requirements including:

  • Ascension Color Vision test (updated every 3 years)
  • Ascension Core Competencies (HIPAA, Joint Commission, OSHA topics; updated yearly)
  • Ascension Onboarding Requirements Education Attestation
  • Ascension Nursing Contract Worker Onboarding
• Immunizations and Health Requirements:

  • MMR: Proof of two vaccines or positive IGG titer (no declinations accepted)
  • Hepatitis B: Vaccination series completion or signed declination
  • Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions require mask wearing
  • Varicella: Proof of immunity or two doses of vaccine (no declinations accepted)
  • Tdap: Valid within 10 years (no declinations accepted)
  • TB Testing: Positive IGRA or PPD results, negative chest X-ray within 1 year, and annual TB questionnaire
  • Fit Test: Required for all travelers, updated annually
• Health Statement or physical exam prior to first assignment, updated annually
• Drug Screening: 12-panel drug screen within 30 days prior to start, updated annually
• Criminal Background Check and Employment Verifications:

  • Criminal background check including SSN trace, OIG search, National Sex Offender, and others within 30 days prior to start
  • EPLS check within 90 days prior to start
  • Employment verification for the past 7 years
  • Education verification within 90 days prior to start
• Professional References: At least two professional references prior to first assignment
• Driver’s License/State ID: Must be current and valid
