Immunization & Health Requirements

- Positive MMR titers within last 15 years or appropriate booster schedule
- Positive Varicella titers within last 15 years or appropriate booster schedule
- Hepatitis B: Proof of full series, partial series with declination, or positive titers within 15 years
- Tdap vaccination within last 10 years (no declinations accepted)
- Influenza vaccine required during flu season (Oct-Mar) or completion of specific declination and masking forms
- 2-Step PPD or IGRA (Quantiferon/Tspot) test with negative results within last 6 months; if positive, clear chest x-ray and TB questionnaire required
- Fit test for respirators completed within 1 year prior to start
- Physical exam within 1 year prior to start confirming ability to work without restrictions
Background & Verification Requirements
- Completed I-9 or attestation of eligibility to work
- Social Security trace and criminal background check within 6 months prior to start (including national sex offender registry)
- Employment verification for last 5 years through third-party verification
- Education verification through third-party; international education requires equivalency documentation
- OIG exclusion search and SAM/EPLS/GSA/OFAC clearance within 6 months prior to start
- Drug screen (10-panel) within 60 days prior to start; instant drug screens not accepted
- National database criminal background check for Ohio residents
Competency & Training Requirements
- UHS Competency Exam for relevant department with minimum 80% score, completed within 1 year prior to start
- UHS Core Competencies training completed within 1 year prior to start (includes HIPAA, OSHA, Joint Commission topics)
- UHS Skills Checklist completed within 1 year prior to start (clinical skill-specific)
- Completion of “Non Employee Required Safety and Compliance Training” modules prior to clearance; renewal annually
