Job Opportunity: Diabetes Care Registered Nurse (RN)
Location: Brooklyn, New York
Employment Type: Clinical - Nursing - DPC
Contract Length: 13 weeks
Job Summary
The Diabetes Care RN plays a key role in the holistic management and education of patients with diabetes. This position requires expertise in diabetes management, including medication knowledge, device troubleshooting, and patient education, working collaboratively with the diabetes care team to provide personalized care.
Key Responsibilities
- Counsel patients on diabetes medications, administration techniques, side effects, and expected outcomes.
- Prepare prior authorizations and appeal medication denials to ensure timely patient access.
- Collaborate with pharmacies and endocrinologists to resolve medication coverage issues.
- Educate patients on diabetes fundamentals, biological and practical management.
- Tailor patient education to individual needs, including culturally sensitive nutritional counseling and exercise guidance.
- Conduct glucose tolerance tests ensuring safety and accuracy.
- Instruct and troubleshoot continuous glucose monitors (CGMs) and insulin pumps, including device integration and data sharing.
- Provide training on insulin administration, blood sugar management, sick day rules, and glucagon pen usage.
- Coordinate with pump distributors for authorization and ongoing supply management.
- Respond to medical inquiries related to diabetes management.
- Collaborate with the diabetes care team, including dieticians, health coaches, and depression care managers, to address diabetes distress and ensure comprehensive care.
Required Qualifications
- Education: Bachelor of Science in Nursing (BSN)
- Licensure: Current and valid New York State Registered Nurse License
- Certifications: Certification in Diabetes Education
- Experience:
- Proven experience with technical aspects of diabetes care, including insulin pumps and CGMs
- Relevant experience in diabetes education
- Proficiency in electronic health records and medical software
- Skills: Strong interpersonal and communication skills with a focus on patient-centered care
Preferred Qualifications
- Master’s degree in Nursing or related field
