- Social Security Number Trace (last 7 years of addresses/employment/school)
- National Sex Offender Check (within 30 days of start, updated annually)
- Exclusion Checks (OIG, GSA, Medicaid exclusion lists)
Additional Preferred/Optional Credentials
- Hepatitis B vaccination or proof of immunity (strongly preferred)
- Tdap vaccination within 10 years (preferred)
- Positive PPD with negative chest X-ray documentation (if applicable)
