Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Personal Health History Requirements

  • MMR (Proof of 2 vaccines or positive titer)
  • Fit Test (Annual respirator fit test; must be completed prior to start if not available onsite)
  • Hepatitis B (No declinations accepted)
  • Influenza Vaccine (Flu vaccine or declination required for Nov 1, 2025 - Mar 31, 2026; no declinations accepted)
  • TB Test (Within one year of start date)
  • Varicella (Vaccination or titer showing immunity)
  • Tdap (No declinations accepted)
  • Physical/Health Statement (Annually; must state clear to work)
  • Prime Drug Screening (Minimum 10-panel test including THC, within 60 days of start; CA locations require additional saliva THC test)
  • Positive PPD - Negative Chest X-Ray (optional)
  • Name Verification Affidavit (optional, if name discrepancies exist)

  • Prime Color-blind Test (Annual; Relias accepted)
