Licensed Practical Nurse (LPN) - Travel/Contract
Location: Augusta, Georgia

Job Description
- Provide and coordinate home nursing care according to physician orders and agency policies.
- Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits and periodic office meetings.
- Average patient load: 5-8 patients per day.
- Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
- Schedule: Monday to Friday, 8:00 AM to 5:00 PM.
- Patient visits may be in both care facilities and patients' homes.
- Must have reliable transportation and a current driver's license.
- Must be willing to travel up to 75 miles from the home clinic.
- On-call requirement: Approximately 1 weekend per month, varies by location.
- Local candidates accepted.
Required Qualifications
- Valid LPN / LVN license in Georgia
- Minimum 1 year of recent Home Health experience, including experience with adult patients
- Minimum 1 year of Hospice experience within the last year
- Recent specialty experience in Home Health and Hospice settings required
- BLS certification issued by the American Heart Association or American Red Cross
- Experience working with HomeCare HomeBase EMR system
Additional Requirements
- Must complete and maintain all required background checks including but not limited to:
- 7-year standard background check
- EPLS/GSA/SAM checks
- State-specific criminal and abuse registry checks as applicable
- Fingerprinting as required by state regulations
- Health documents required prior to start:
- Physical exam within 12 months
- Negative TB test or chest X-ray within 12 months
- Proof of immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella
- Drug screening (10 panel)
- Flu vaccination or declination for current season
- COVID vaccination series or declination
- Must review and sign all compliance, confidentiality, and policy documents prior to start.
- Must have personal or auto insurance policy under candidate’s name or list candidate as a driver.
Work Environment
- Dress code: Scrubs
- Documentation and charting primarily through HomeCare HomeBase and Oasis EMR systems.
- Work involves travel to patient homes and occasional visits to care facilities.
Notes
- Contract may have possibility of extension or permanent hire at assignment end.
- Weekend and on-call requirements vary; discuss during interview.
- Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient not reimbursed).
