Location: Santa Rosa, California
Contract Length: 13 weeks

Specialty: Home Health Therapy/Rehabilitation
Shift: Day shift, Monday to Friday (5x8 hours), 8:00 AM to 4:30 PM
Guaranteed 40 hours per week
Average patients per day: 5-7 (to be confirmed during interview)
Minimum of 30 OASIS points weekly required
Types of cases: Geriatric
Territory: Up to a 60-mile radius from the office; flexibility with territory expected
Dress code: Scrubs of any color; no jeans allowed
Charting must be completed by 9:00 AM the following morning
Timely communication required
iPad provided for documentation; cell phone use not allowed during work
Mileage reimbursed at IRS standard rate
Locals accepted at travel rate
On-call required (details to be discussed during interview)
First-time travel therapists not accepted

Valid Physical Therapist license in California
Minimum 1 year of recent Physical Therapist experience within the last year
BLS (Basic Life Support) certification required (AHA, ARC, or Military accepted)
Experience with OASIS documentation and processes
