Job Details

- Start Date: ASAP
- Shift: 3 shifts of 12 hours each (3x12s)
- Guaranteed Hours: 36 hours per week
- Weekend Requirement: Every other weekend and every other holiday
- Floating: Required as per facility needs
Experience Requirements
- Minimum Years of Experience: 1 year
- Recent Experience: Required within the past year
- Prior Travel Experience: Not explicitly required
Licenses and Certifications
- License Required: New York State LPN/LVN License (or proof of application)
- Certifications Required:
- ACLS (Advanced Cardiac Life Support)
- BLS (Basic Life Support)
- NIHSS (NIH Stroke Scale)
Required Skills and Competencies
- MS/Telemetry experience
- Interpretation and management of dysrhythmias
- Use of Rapid Response/Code teams
- Peripheral line care/management
- Nasal cannula and oxygen administration (including oxygen tanks)
- Non-invasive airway management
- Isolation precautions
Orientation and Testing
- Must pass a telemetry assessment (minimum 80%) on the first day of orientation if assigned to telemetry-related units
- Medication and pharmacology competency exams as applicable
- Facility-specific orientation and safety manual acknowledgment required
Background and Health Requirements
- Background check within 1 year covering all locations lived/worked/schooled in the past 7 years
- EPLS/GSA/SAM and HHS/OIG checks prior to start
- Physical exam within 12 months prior to start, signed by a licensed provider
- Fit mask test within 12 months, updated annually
- Immunizations and health screenings required:
- Tetanus/Diphtheria/Tdap (within last 10 years or declination)
- Negative TB test or chest X-ray (within 1 year)
- Measles, Mumps, Rubella immunity (2 MMR vaccines or serologic titers)
- Hepatitis B series, titer, or declination
- Varicella immunity (vaccines, positive titer, or documented history)
- Seasonal flu vaccination or declination (during flu season)
- COVID vaccine records or declination
- 9-panel drug screen within 30 days of start
Documentation and Compliance
- Must provide unexpired government-issued photo ID
- Nursys license verification prior to start
- State license primary source verification prior to start
- Required to review and sign facility-specific documents including:
- Code of Conduct
- Patient Abuse Reporting
- Confidentiality and Non-Disclosure Agreement
- Orientation Packet Acknowledgement
- Automated Dispensing Cabinet Security (for Pyxis access)
- Annual Tuberculosis Screening Questionnaire
Additional Notes
- Call-off policy allows 3 shifts off per 13-week assignment
- Any requests for time off (RTO) must be submitted with application; no RTO approved after offer
- No hits on nursing license approved (automatic disqualifier if issues found)
