Job Opportunity: Certified Occupational Therapy Assistant (COTA) - SNF

Location: Live Oak, California
Employment Type: Travel/Contract
Contract Length: 13 weeks (approximately 3 months) preferred
Start Date: ASAP (flexible for later start dates)
Shift Schedule:

• 5 days per week, 8-hour day shifts
• May include occasional weekend coverage as needed
  Guaranteed Hours: 36 hours per week

Specialty & Experience Requirements

• Specialty: Therapy/Rehabilitation (Certified Occupational Therapy Assistant)
• Required License: Active California COTA license in hand
• Experience:

  • Minimum 1 year of experience in Skilled Nursing Facility (SNF) preferred
  • New graduates will be considered
• Prior travel experience: Not explicitly required

Certifications & Competencies Required

• ACLS (Advanced Cardiac Life Support) from American Heart Association or American Red Cross
• BLS (Basic Life Support) from American Heart Association or American Red Cross (current, clear copy required)
• PALS (Pediatric Advanced Life Support) preferred
• National Tech Certification as applicable
• State License verification (primary source online verification)
• Medication Test/Pharmacology Competency Exam (as applicable)
• Competency Exam (as applicable)

Health & Background Requirements

• Physical exam within 1 year of start date
• Negative TB test (PPD, Quantiferon Gold, or T-spot) within 1 year of start; if past positive, negative chest x-ray within 1 year required
• Immunizations or proof of immunity for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (titer, declination, or vaccine series)
  • Varicella (titer, vaccines, or history of chickenpox)
• 9 Panel Drug Screen within 1 year of start (instant drug screen acceptable)
• Seasonal Flu Vaccination or Declination during flu season (Oct 1 - Mar 31)

Additional Requirements

• Background checks including 7-year search of locations lived/worked/schooled, sex offender registry, EPLS/GSA/SAM, and HHS/OIG (updated annually)
• Emergency contact form required prior to start
• 2 professional references required for travel, allied, or per diem acute care roles (within 3 years of start)
• Annual skills checklist required
