Job Description

The Case Manager (Discharge Planner) supports the Case Management team by facilitating safe and effective discharge plans for patients. This role involves coordinating post-acute care, durable medical equipment (DME), community resources, and transportation. The Case Manager collaborates with physicians, nurses, patients, families, and insurance payors to ensure continuity of care and appropriate utilization management.

Key Responsibilities

• Assist clinical team in securing post-acute care, DME, community resources, and transportation.
• Coordinate communication among physicians, clinics, patients, families, and Case Managers.
• Provide timely and accurate information to payors and service partners.
• Support utilization management and discharge planning functions.
• Research and negotiate patient needs during discharge.
• Collaborate with third-party payer case management for authorizations and documentation.
• Facilitate referrals for home health care, hospice, medical equipment, and transfers.
• Document discharge planning information in the EMR according to standards.
• Arrange follow-up appointments to prevent readmissions.
• Advocate for patients’ rights and consult with social workers for social service needs.
• Review insurance authorizations and clinical data to support utilization review.

Required Qualifications

• Valid LPN / LVN license in Oregon
• Minimum of 1 year of Case Management experience, with experience within the last year
• ACLS issued by the American Heart Association
• BLS issued by the American Heart Association
• PALS issued by the American Heart Association

Compliance & Background Requirements

• 7-year criminal background check within 30 days of start; updated annually.
• EPLS/GSA/SAM and HHS/OIG checks required 30 days prior to start and updated monthly.
• Education verification required for all clinical roles.
• International background check if residency outside the US in past 10 years.
• Physical exam within 1 year prior to start confirming good physical and mental health.
• Drug testing: 10-panel including methamphetamines within 30 days prior to start.
• Fit mask testing onsite on first day; must fill out respiratory questionnaire prior to start.
• Immunizations and titers required or documented declinations accepted for:

  • Tdap (within 10 years)
  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B (immune titer and/or vaccine series or signed declination)
  • Varicella
• Seasonal flu vaccination or signed declination required during flu season.
• TB testing: Qgold/TSPOT within 1 year prior to start (PPD not accepted); annual TB questionnaire for past positives.

Additional Facility Requirements

• Completion of facility-specific documents including confidentiality agreements, vaccine declination/waiver forms if applicable, and orientation safety manual acknowledgment.
• Photo submission for badge issuance prior to start.
• Availability for compliance needs up to start date (notify if unavailable due to vacation or travel).
• At least 2 professional employment references required.
• Skills checklist required.

Schedule

• Typical hours: 8 AM – 5 PM
• Flexible schedule options: Monday-Friday or Tuesday-Saturday
