Schedule

• Monday through Friday
• Day shift, 5x8 hours
• No weekends

Assignment Details

• Home health setting
• Geriatric patient population
• Average census: 5–7 patients per day
• Guaranteed 30 visits weekly with a 40-hour weekly guarantee
• Territory may include up to a 60-mile radius, including parts of Walla Walla, Columbia, and Umatilla counties
• First-time travelers are not accepted
• Local candidates are accepted at travel rate

Required Qualifications

• Valid Physical Therapist license in Washington
• Physical Therapist (1 year of experience)
• Physical Therapist experience within the last year
• One completed travel assignment
• BLS issued by American Heart Association or American Red Cross
• Driver's License
• Experience with Homecare Homebase
• Experience with OASIS

Compliance / Onboarding

• Background check and state-specific screening requirements apply
• Education verification required
• Physical exam within the last 12 months
• Fit test required
• Negative TB test or chest X-ray required
• TB questionnaire required annually
• Measles, mumps, rubella, hepatitis B, varicella, flu, and COVID documentation or declination requirements apply
• Drug screen within the last 12 months
• 2 professional references required
• 7 years of work history must be verified
• Full 7-year work history must be listed in the application with no gaps
• Skills checklist required annually
• Personal auto insurance required because the role involves driving

Work Expectations

• Timely documentation and communication are required
• All charting must be completed by 9:00 a.m. the following morning

Equipment / Logistics

• iPad provided
• Cell phone not provided
• Mileage reimbursed at the IRS standard rate

Dress Code

• Scrubs of any color
• No jeans
