Contract Length: 13 weeks
Position Details
- Specialty: Therapy/Rehabilitation (Physical Therapist Assistant - Home Health)
- Shift: Monday to Friday, 8:30 AM to 5:00 PM (40 hours/week)
- Guaranteed hours based on hours signed
- Local candidates accepted within a 75-mile radius
- Must have Home Health experience (preferred but all PTA experience considered)
- Attendance policy: More than two missed shifts without reasonable explanation during the 13-week contract may lead to contract review and possible cancellation
Required Qualifications
- Valid Physical Therapy Assistant license in South Carolina
- Minimum of 1 year of Physical Therapy Assistant experience, including experience within the last year
- Experience in a Home Health setting
- Basic Life Support (BLS) certification required (AHA or RQI acceptable)
Background and Compliance Requirements
- 7-year criminal background check within 1 year of start, updated annually, covering all locations lived/worked in past 7 years
- EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start and updated for contract renewals
- Education verification required (highest level of education must be associate degree or higher; certificates alone are not sufficient)
- Social Security card required if used for I9 documentation
- Drug screening within 90 days of start (10-panel plus Oxycodone and Marijuana)
- Physical exam and fit mask (N95) within 1 year, updated annually
- Negative TB test or chest X-ray within 3 months of start (specific acceptable tests detailed)
- Required immunizations or proof of immunity (Measles, Mumps, Rubella, Varicella, Hepatitis B) or approved medical/religious exemptions with masking requirements
- COVID vaccination (dose 1 and 2) required or declination form with masking
- Seasonal flu vaccination or declination required during flu season (Nov 1 - Mar 31) with masking if declined
Certifications and Competencies
- BLS certification required for all candidates
- Orientation Safety Manual acknowledgment required
- Medication/Pharmacology competency exam required within 1 year and updated annually
- Facility-specific documents including photo for badge and TB risk assessment required
- Pass/fail testing and additional certifications (e.g., ACLS, PALS) not required for this role
Additional Requirements
- Must submit all requested Return to Office (RTO) restrictions with initial submission; late requests will not be considered
- Two professional references required within 1 year of start (Qualivis requirement)
- Employment application required
- Skills checklist must be updated annually
