Job Opportunity: Medical Surgical Nurse (LVN / LPN) - Travel/Contract

Location: Hinton, West Virginia
Contract Length: 13 weeks
Shift: Night (3 or 4 shifts per week, 12 hours each, 7:00 PM – 7:30 AM)
Hours per Week: 36 hours

Job Description

• Minimum 2 years of current hospital experience as an LVN/LPN required.
• Patient ratio approximately 1:6.
• Experience with Meditech EMR system preferred.
• Must be able to float within scope of practice.
• Every other weekend required.
• Blocked scheduling is not accommodated.
• Orientation occurs bi-weekly.
• Candidates must reside within 75 miles of the facility.

Required Qualifications

• Valid LPN / LVN license in West Virginia.
• Minimum 2 years of LPN / LVN experience, including experience within the last year.
• Experience in Medical-Surgical nursing.
• Current Basic Life Support (BLS) certification issued by the American Heart Association.

Certifications & Competencies

• Basic Life Support (BLS) - AHA Instructor/Provider/RQI.
• Medication Test/Pharmacology Competency Exam.
• Competency exams relevant to specialty, renewed annually.
• Dysrhythmia/EKG Competency Exam (required for all units except Psych, OR, Rehab, and true Med Surg).
• Core Competency Parts I, II, and III.
• Orientation Safety Manual Acknowledgement.
• National certification verification as applicable.
• State license primary source verification prior to start and upon expiration.
• NURSYS verification prior to start.

Health & Safety Requirements

• Annual physical exam or physician note within 1 year prior to start, stating clearance to work without restrictions.
• Fit mask test to be completed onsite; PAPRs not accommodated if facial hair present.
• Valid Tetanus/Diphtheria/Tdap vaccination (Td not accepted); declinations accepted if within 1 year of start.
• Negative TB test via QuantiFERON blood test within 1 year prior to start; T-Spot/PPD not accepted.
• TB questionnaire required if TB positive.
• Proof of immunity or vaccination for Measles, Mumps, Rubella (2 MMR doses or positive titer).
• Hepatitis B series, titer, or declination (declination must be within 1 year of start).
• Varicella immunity proof (2 vaccinations or positive titer).
• Drug testing within 30 days of start using specified 9-panel bundle (no THC) with expanded opiates and oxycodone.
• Seasonal flu vaccination or approved medical/religious exemption required during flu season; exemption requests must be submitted within 48 hours of offer acceptance.

Background & Compliance

• Comprehensive background check including:

  • National criminal search, SS & address trace, sex offender, fugitive, violent offender databases.
  • County searches for all counties in last 7 years.
  • FACIS 3 or equivalent, SAM (EPLS), OIG, OMIG, OFAC.
  • Social Security verification.
• Education verification included in background or third-party verification.
• State-specific clearances (e.g., West Virginia APS/CPS search) with annual updates.
• Drug tests and background checks must be run through specified vendor/package.
• Employee health attestation and compliance with facility-specific health documentation and policies.
• Fit test policy and mask requirements must be reviewed and followed.

Additional Notes

• Candidates must sign assignment confirmation within 24 hours of offer acceptance.
• Only one job submission allowed at a time.
• Candidates must provide unexpired government-issued photo ID.
• Candidates must complete all required documentation and attestations prior to start.
• Facility accommodates candidates with latex allergies but requires notification prior to start.
• Scheduling follows a 36/48 hour pay period schedule unless otherwise noted.
