Job Opportunity: Registered Nurse – Care Coordination/Case Management

Location: Carmichael, California
Contract Length: 13 weeks
Shift: Days (8 hours per day, approximately 40 hours per week)
Work Schedule: Every other weekend (at least one full weekend per month), holidays not required but may be worked if willing

Job Description

Seeking an experienced Care Coordinator for a travel contract in a hospital setting. The role involves both discharge planning and utilization management across all hospital areas (no outpatient or home care). Typical caseload is 15-20 patients/consults per day.

Required Qualifications

• Valid RN license in California
• Bachelor of Science in Nursing (BSN)
• Minimum 2 years of case management experience, including experience within the last year
• Experience in utilization management (UM) and utilization review
• Experience with discharge planning
• BLS certification

Orientation & Training

• 5 days of care coordination new employee orientation
• Additional unit-specific orientation as needed

Work Environment & Additional Information

• Business casual attire required
• Hospitalist and intensivist coverage in-house
• Respiratory therapy available 24/7; pharmacy not available 24/7
• Use of Cerner EMR system
• Position includes both utilization review and discharge planning responsibilities
• Candidates must provide 2 supervisory references (one within the past 12 months, one within the past 3 years)

Patient Population & Duties

• Manage cases involving all payers, including uninsured and homeless patients
• Complete initial assessments, Interqual reviews, and discharge planning
• Collaborate with multidisciplinary teams to ensure effective patient care coordination

This position offers a structured day shift schedule with a focus on comprehensive case management in a hospital setting.
