Job Opportunity: RN Case Manager

Location: Brooklyn, New York


Job Summary

Coordinate and ensure delivery of patient care, collect utilization management and quality assessment data, interact regularly with physicians to clarify plans of care, and review patients for high-risk criteria related to discharge planning needs.


Responsibilities

  • Monitor patients and determine needs for timely discharge
  • Confer with third-party payers within 24 hours of notification
  • Conduct initial case review within 72 hours of admission
  • Screen patients for high-risk discharge planning criteria
  • Develop discharge plans for uncomplicated cases
  • Refer to Social Worker for assessment and discharge planning as needed
  • Collaborate with healthcare team members to facilitate care progress
  • Ensure attending physician develops a timely, objective-oriented plan of care
  • Negotiate care plans with physicians and healthcare practitioners as needed
  • Monitor duplication of tests and delays in diagnostics, treatment, or scheduling
  • Ensure documentation of patient progress toward clinical outcomes
  • Conduct concurrent reviews and participate in performance improvement data collection
  • Review cases for continued stay appropriateness and collaborate with physicians on issues
  • Document review findings and provide clinical information to third-party payers
  • Perform retro reviews as requested by third-party payers
  • Maintain current knowledge of field developments through literature, education, and professional organizations
  • Maintain high levels of patient and staff satisfaction with professionalism and diplomacy
  • Assist colleagues and other stakeholders in completing tasks or obtaining assistance
  • Perform other duties as assigned or necessary in emergencies

Contract Length

  • 13 weeks

Required Qualifications

  • Minimum 3 years of direct patient care experience
  • Bachelor’s Degree in Nursing (BSN) required
  • Current New York State RN License
  • Basic Life Support (BLS) certification
  • Advanced Cardiovascular Life Support (ACLS) certification

Preferred Qualifications

  • Master’s Degree in Nursing
  • Pediatric Advanced Life Support (PALS) certification
  • Case Manager Certification (CCM)

Flexible even when you call out so unorganized can't keep track of staff. Probably wouldn't work here if it wasn't very high pay

Cons

Very high ratios like 6-12 on med surg.. very unorganized and hot.. some nurses and Cna's are rude
