Contract Length: 13 weeks

Job Description

• 1 year of transplant experience required
• Inpatient pharmacy experience required
• Experience with EPIC electronic health record system required
• Weekend rotation required
• Board Certified Pharmacist (PharmD) required
• Must have Louisiana pharmacist license in hand at time of application

Required Qualifications

• Valid Pharmacist license in Louisiana
• PharmD degree
• At least 1 year of pharmacist experience, including experience within the last year
• Experience working in an inpatient setting
• Proficiency with Epic electronic health record system

Compliance & Documentation Requirements

• Government-issued photo ID matching candidate documentation
• Background check within 6 months covering all locations lived/worked/schooled in past 7 years
• Various federal and state background screenings (e.g., EPLS, HHS/OIG, Sex Offender Search)
• Drug screening within 30 days of start date (comprehensive panel including marijuana, cocaine, amphetamines, opiates, etc.)
• Immunization and health documentation including:

  • Tetanus/Diphtheria/Tdap (within 10 years)
  • Negative TB test or chest X-ray (recent testing required)
  • MMR (Measles, Mumps, Rubella) vaccination or positive titer
  • Hepatitis B series, titer, or declination
  • Varicella vaccination or positive titer
  • Seasonal flu vaccination or approved exemption/declination (required Oct 1 – March 31)
• Physical exam documentation (within 12 months)
• Employment application with 7 years of work history, no gaps over 30 days
• Annual competency exams including medication/pharmacology competency (score 80% or higher)

Additional Notes

• Candidates must be prepared for weekend rotations as part of the schedule
• Board certification and active state licensure are mandatory before submission
• Compliance with all background checks, drug testing, and immunization requirements is required prior to start
• Competency exams and certifications must be maintained and updated annually
