Infusion experience within the last year
1 year of infusion experience
Excellent IV placement skills
Will place IVs, draw labs, administer blood products, and hydration
Will not administer chemotherapy
BLS - Basic Life Support
ACLS - Advanced Cardiac Life Support issued by American Heart Association
State license verification
Criminal background check within 6 months of start, including:
- 7-year search
- all alias searches
- county searches
- National Sex Offender search
EPLS/GSA/SAM run within 30 days of start and updated monthly
HHS/OIG run within 30 days of start and updated monthly
Color blind testing
Unit-specific competency exam within 3 years of start and updated every 3 years
Core Part I, Core Part II, Core Part III
OSHA Mandatory
Physical exam within 1 year of start
N95 mask fit testing within 1 year of start
Tetanus/Diphtheria/Tdap within 10 years of start
Negative TB screening or chest X-ray
TB questionnaire
