Required Credentials and Documentation
Licensing & Certifications
- Must have LCSW license in hand
- ACLS, BLS, PALS, NRP, CPI, and other certifications as applicable (verify per clinical track)
- State License verification and NURSYS verification required for RNs/LPNs as applicable
- IV Certification, Management of Assaultive Behavior (MAB), NIH Stroke Certification, Chemotherapy, Electronic Fetal Monitoring (EFM) certifications as applicable
- Certification requirements depend on clinical track and facility policies
Background Checks & Verifications
- Background check within 45 days of start, covering all locations lived/worked in past 7 years; updated annually
- EPLS/GSA/SAM and HHS/OIG checks within 30 days of start; updated monthly
- Education Verification required for all candidates
- State-specific searches (Oregon Abuse/Neglect, Washington WSP/WATCH, OBRA Registry for CNAs in WA) as applicable
- Drug testing: 10-panel drug screen within 90 days of start (no instant tests)
- References: Minimum 2 supervisor references covering 1 year employment within last 3 years (6 months for CNAs)
- Employment application covering at least last 5 years (7 years for APN/Subcontractors)
Health Requirements
- Physical exam within 1 year of start, signed by MD, NP, PA, or DO
- Fit mask test (travel only, new starts after 2/5/2024), updated annually
- Immunizations/vaccinations:
- TDAP (within 10 years) or declination
- Negative TB test or chest X-ray within 12 months, updated annually
- MMR (Measles, Mumps, Rubella) - 2 vaccines or positive titer or declination
- Hepatitis B vaccination or signed declination (special rules for Alaska placements)
- Varicella - 2 vaccines or positive titer or declination
- Seasonal Flu vaccine or declination (required during flu season)
- COVID vaccine or declination
- Respiratory Safety Clearance (travel only, new starts after 2/5/2024), annually
Competency & Skills Requirements
- Medication Test/Pharmacology Competency Exam as applicable
- Competency exams required by profession/specialty
- Color Blind Testing if specifically requested
- Skills checklists updated annually for applicable units
- Medication Aide exam required for CNAs with LTC specialty at certain Oregon Assisted Living facilities
Facility Specific Requirements
- Floating Policy Acknowledgement: Traveler may be required to float to other Providence facilities within reasonable distance
- Badge photo (passport style with neutral background) required
- Completion of Providence WorkRamp training modules prior to start, including:
- Core modules by profession and region
- Specialty-specific modules for ER, L&D, leadership roles
- Annual regulatory modules
- Regional specialty modules as applicable
- Epic training modules required; transcripts must be submitted and validated prior to working
- Providence Health and Services system TB questionnaire required prior to start and updated annually
Additional Notes
- Travelers must comply with all Providence Health and Services policies and documentation requirements
- Approval for time off and scheduling is at the discretion of the client
- Returning candidates with less than 30-day gaps may have some background and drug screen requirements waived if previously completed at Providence facilities
- Some certifications and requirements may be waived for non-clinical or remote workers
