Job Requirements & Details
- Must have home health and OASIS experience
- Locals allowed at the same rate
- Basic Life Support (BLS) certification required
- Work schedule: Monday to Friday with every 4th weekend rotation
- 6 points per day (shift credit system)
Required Certifications & Competencies
- BLS (Basic Life Support): Current certification required; online CPR not accepted
- ACLS, PALS, NRP, EFM: Required depending on unit; confirm with program management if unclear
- NIH Stroke Certification: Required for all inpatient RNs except OR and PICC; must be updated annually
- Medication Test/Pharmacology Competency Exam: Must be completed within one year of start date
- Competency Exams: As applicable to profession/specialty, completed within one year of start date
- Acknowledgement of Pass/Fail Testing: Required for all clinical positions
- Orientation Safety Manual Acknowledgement: Required
- Core Parts I, II, III: Required for subcontractors or as applicable
Background & Screening Requirements
- 7-year criminal background check within one year of start, updated annually
- Must include all locations lived/worked in past 7 years
- Social Security trace, county, statewide, nationwide, sex offender checks required
- DMV driving record required for Home Health & Hospice positions (minimum 3 years driving experience, minimum age 21; 25 if transporting patients)
- Virginia State Police Check required prior to start for Home Health and Lifelong Care positions
- Fingerprinting via Virginia State Police National Criminal Background Check (effective 7/15/2025)
- Drug screen within last 12 months (specific panels required)
- Physical exam within last 12 months
- Negative TB test or chest X-ray within last 12 months; if positive, clearance letter required
- Flu vaccination required during flu season (10/1 - 3/31) with hard documentation deadline; medical/religious exemptions accepted with approval
- COVID-19 vaccination preferred but not required; if vaccinated, documentation or declination form required
Health & Immunization Requirements
- Rubeola (Measles): 2 vaccines or positive titer; exemptions apply for candidates born before 1957
- Mumps: 2 vaccines or positive titer; exemptions apply for candidates born before 1957
- Rubella: 2 vaccines or positive titer; exemptions apply for candidates born before 1957
- Hepatitis B: Series, titer, or facility-specific declination
- Varicella: 2 vaccines, titer, or history of chickenpox; exemptions require approval
Facility Specific Requirements
- Photo for badge (recent, professional, neutral background) required 5 days prior to start
- Confidentiality agreement, scrub colors, safety brochure, dress code policy, drug-free policy, hand washing hygiene policy must be reviewed and signed prior to start
- Lifelong Care/Home Health/Hospice candidates must complete sworn disclosure within 30 days of start and update annually
- TB questionnaire required annually
- References: 2 professional references required
- Employment application/resume required
- Skills checklist must be updated annually
Additional Notes
- Candidates must have a valid and current government-issued photo ID and driver's license
- Candidates must be at least 21 years old with minimum 3 years driving experience; 25 years old if transporting patients
- Pass/fail computer-based learning (CBL) tests administered at orientation; multiple attempts allowed
- Stroke education CEUs required annually for acute care hospital RNs, hours vary by unit
