Case Manager (Registered Nurse)
Location: Atlanta, Georgia Employment Type: Travel/Contract Contract Length: 7 weeks Shift: Day (5x8 hours), 8:00 to 16:30 Hours: 8 per day, 40 per week
Schedule and Assignment Details
- Monday to Friday schedule
- No weekend shifts
- No on-call shifts
- Floats within the facility and scope of practice as needed
- Maximum of 7 days of requested time off (RTO); discuss requests during the interview. No time off requested for September.
- Holiday coverage: If one major holiday falls in the assignment, the traveler must work it. If two fall in the assignment, the traveler must work both. If three fall in the assignment, the traveler must work two of them.
- Major holidays: Memorial Day, Independence Day, Labor Day, Thanksgiving/Black Friday, Christmas Eve/Christmas Day, New Year's Eve/New Year's Day
- Locals accepted
- Orientation hours equal the contract length in hours (for example, 8 hours for an 8-week assignment).
Role Responsibilities
- Assess, evaluate, and drive the discharge plan
- Coordinate services to the patient's appropriate level of care
- Ensure the treatment plan is initiated, evaluated, and re-evaluated daily
- Follow patient progress daily and monitor length of stay
- Identify barriers to discharge
- Follow Medicare regulatory guidelines regarding patient choice, IMM notices, and MOONS
Required Qualifications
- Valid RN license in Georgia
- Case Management (3 years of experience)
- Case Management experience within the last year
- One completed travel assignment
- Associate of Science in Nursing / Associate Degree in Nursing
