Hospice RN - Pennsylvania (Irwin, PA)

Job Summary

  • Provide direct patient care services to hospice patients and supportive care to their families.
  • Manage an interdisciplinary team including Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff to enhance hospice care quality.
  • Case manage all assigned patient clinical records.
  • Perform initial and ongoing patient assessments.
  • Develop and implement the Hospice Plan of Care in collaboration with physicians and the interdisciplinary team.
  • Deliver comprehensive physical, psychosocial, educational, emotional, and spiritual services, including bereavement support.
  • Report changes in patient condition to attending and hospice physicians.
  • Maintain accurate and updated medical records, including interdisciplinary notes and plans of care.
  • Practice infection control measures and standard precautions.
  • Monitor staff scheduling and participate in quality assessment and performance improvement programs.
  • May participate in agency on-call schedule.
  • Maintain knowledge of pain and symptom management in the dying process.
  • Conduct routine supervisory visits with team members at least monthly.
  • Review and follow up on patient clinical records to ensure quality and compliance with policies and regulations.
  • Maintain close contact with patients’ families to provide information, support, and continuity of care.
  • Act as primary liaison between physician, patient/family, and hospice team.
  • Educate patients, families, and caregivers on care needs including medication administration, dressing changes, oxygen use, hospice philosophy, and care of the terminally ill.

Contract Length

  • Approximately 13 weeks (89 days rounded to 13 weeks).

Required Skills & Experience

  • Minimum of 1 year RN experience in acute care hospital, oncology, long-term care, hospice, or home health (school internships and clinical hours may be considered).
  • Knowledge of terminally ill patients and hospice care needs.
  • Understanding of roles of all hospice disciplines and excellent patient assessment skills.
  • Familiarity with community resources, strong oral and written communication, documentation, and program management skills.
  • Knowledge of general nursing practice, managed care principles, and regulatory guidelines (Medicare, Medicaid, ACHC).
  • Ability to operate nursing assessment tools and common office equipment.
  • Must have and maintain transportation for work.
