Overview
- Day shift, 8 hours per day, 40 hours per week
- 13-week contract
- Billings, Montana
Required Qualifications
- Valid RN license in Montana
- Bachelor of Science in Nursing
- 3 years of Case Management experience
- Case Management experience within the last year
- Experience in an Acute Care setting
- BLS
Preferred Qualifications
- 5 years of nursing experience
- ACM or CCM certification
- Hospital care management experience
Role Focus
- Coordinates care and services for patients needing assistance or meeting Care Management criteria
- Coordinates discharge planning
- Assesses patient needs, functioning, support systems, and community resource access
- Collaborates with physicians and the care team on care planning, utilization, and discharge needs
- Participates in rounds, case conferences, and multidisciplinary groups
- Advocates for patient rights
- Supports education related to case management and discharge planning
