Overview

• Travel assignment in Medford, Wisconsin
• Contract length: 13 weeks
• Guaranteed hours: 40 per week
• Primary location: Aspirus at Home – Medford

Schedule

• Monday–Friday, 8:00 AM–4:30 PM
• Rotates one Saturday every 6–8 weeks
• No holidays
• Can be low censused and put on call; call-back minimum is 2 hours

Float Scope

• Occasional travel to central and northern regions

Required Qualifications

• AHA or ARC BLS
• Wisconsin licensure
• NPI number
• Master’s Degree in Physical Therapy
• Home Health experience
• Experience working with adult and geriatric patients with orthopedic, neurological, and functional mobility diagnoses

Role Details

• Evaluates, treats, and plans patient care in accordance with Physical Therapy standards of practice
• Population served includes adult and geriatric patients
• Documentation is done using NetSmart EMR
• Laptop and cell phone are used for point-of-service documentation, GPS, and communication
• Average productivity expectation is 4.5 visits per day

Additional Notes

• Mileage is reimbursed at the government-approved rate
• Contractors must live outside a 50-mile radius from the facility
• Scrubs: any color or business casual
• During flu season, travelers who decline flu and COVID vaccination must wear a mask at all times
