Licensed Practical Nurse (LPN) - Long Term Care

Facility & Department Overview

• Facility type: Skilled Nursing Facility (SNF) and Long Term Care (LTC)
• Census: Typically 80-95 residents; 20 rooms are not private
• EMR system: PCC
• Patient ratios:

  • RN: LTC 1:25, SNF 1:13
  • LPN: LTC 1:25, SNF 1:13
  • CNA: 1:10-15 (aim to keep around 10)
• Interdisciplinary team includes PT/OT/SLP, Respiratory Therapist, Social Worker, MDS nurses, Wound Care Nurse, Dietician
• Respiratory Therapy available during the day M-F; RT coverage mostly at night for AVAP/CPAP/BIPAP
• Pharmacy on-site, available 24/7 with Omnicell and med carts for medication management

Job Duties & Patient Care

• Provide IV therapy (antibiotics and fluids), assessments, medication administration, dressing changes, and wound vac care
• Admission and discharge processes are completed by RNs
• CNAs handle vitals and activities of daily living (no blood sugars)
• Common patient diagnoses include feeding tubes, IV antibiotics, post-surgical care, rehabilitation, and long-term care needs
• No titratable drips required
• Floating between units possible but generally assigned to the same unit during a stretch
• COVID patients are intermixed with other residents

Scheduling & Work Environment

• Shift hours: 5:00 AM to 5:00 PM
• Weekend and holiday work required every other week
• No call or standby requirements
• Management tries to accommodate block scheduling
• Scrub color: Any professional color accepted
• Orientation: One 12-hour shift on the floor; additional support available if needed
• Use of both PAPR and N95 masks

Required Qualifications

• Valid Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license in Illinois
• Minimum 1 year of Long Term Care experience within the last year
• BLS Certification required
• Driver’s license required for submission
• First-time travelers are welcome; prior travel experience is not required

Additional Notes

• RN attendance at team conferences and grand rounds is encouraged but not mandatory
• Physician coverage includes hospitalist after hours and nurse practitioner during weekdays; 24/7 on-call provider available
• Charge nurses are RNs with dedicated night coverage and full patient loads
