Description
We are seeking an experienced Home Health RN to provide skilled nursing care to patients in their homes and coordinate care with physicians, families, and interdisciplinary teams.
Registered Nurse (RN) – Home Health
Location - Tucson, AZ
Contract length - 13 weeks
Responsibilities
- Perform comprehensive patient assessments and OASIS assessments.
- Develop, implement, and update individualized Plans of Care.
- Provide skilled nursing, wound care, medication management, IV/venipuncture, catheter/drain care, and pain management.
- Educate patients and families on medications, treatment plans, safety, and self-care.
- Supervise HHA/STNA/LVN staff and coordinate patient services.
- Maintain accurate clinical documentation and ensure compliance with payer and regulatory requirements.
- Participate in case conferences and provide on-call support as assigned.
- Travel locally to patient homes and work independently.
Requirements
- Current RN license
- 2+ years of Home Health experience
- OASIS experience – Required
- Homecare Homebase (HCHB) experience – Required
- Current CPR/BLS certification
- Valid driver's license, reliable transportation, and insurance
- ADN/Diploma in Nursing required; BSN preferred
- CHPN certification preferred
