Description
Pride Health is actively hiring a Registered Nurse (RN) to join our client's Home Health Team.
Location: Hot Springs, AR 71913
Pay Rate: $55.00/hour W-2
Schedule: Monday–Friday, 8:00 AM–4:40 PM
Hours: 40 hours/week; 32-hour minimum
Productivity: 30 points/week
On-Call: 1 weekend every 1–2 months, based on agency need
Mileage: Reimbursed between patient visits

Job Summary:
- We are seeking a compassionate and experienced Registered Nurse (RN) to provide skilled nursing care to patients in their homes.
- The RN will coordinate care with physicians, patients, families, and the interdisciplinary team while developing and managing individualized plans of care.
Key Responsibilities:
- Conduct initial and comprehensive nursing evaluations and determine patient eligibility and medical necessity for home health services.
- Develop, implement, and revise individualized Plans of Care (POC) in collaboration with physicians and healthcare professionals.
- Complete accurate OASIS assessments, including medication reconciliation.
- Provide high-quality skilled nursing services in accordance with the plan of care, scope of practice, and infection-control standards.
- Accurately and timely document patient visits and required clinical information in accordance with payer and agency requirements.
- Supervise and provide direction to Home Health Aides and LPN/LVNs as permitted by state and federal regulations.
- Communicate patient needs, changes in condition, orders, scheduling updates, and other relevant information to physicians and agency staff.
- Participate in case conferences, performance improvement initiatives, survey readiness, and utilization management activities.
- Work collaboratively with the interdisciplinary team to improve patient outcomes, safety, and reduce unnecessary hospitalizations.
- Assist with orientation and serve as a preceptor for new staff and students when needed.
- Maintain schedule flexibility to meet patient-care needs, including weekend/on-call rotation and potential holiday coverage.
Required Qualifications:
- Active Arkansas RN License
- Current CPR certification
- Valid Arkansas driver's license
- Reliable vehicle and current auto insurance
- Ability to meet productivity expectations of 30 points/week
- Willingness to participate in the required weekend on-call rotation
Preferred Qualifications:
- Previous Home Health experience
- OASIS experience
- HCHB (Homecare Homebase) experience
