Description
Work Schedule:
100% FTE, day shift working Monday – Friday, 8a – 5p. You will work at the Mile Bluff, Lake Delton and Portage clinics.
Pay:

We are seeking a Nurse Practitioner or Physician Assistant to:
- Participate in multidisciplinary HD rounds/ collaborate with the patient's care team for care coordination.
- Write and manage dialysis orders
- Manage and treat anemia and BMD
- Assess and refer for dialysis access placement or intervention
- Blood pressure management
- Volume status/estimated dry weight assessment
- Lab results review/management
- Referral for transplant and/or home dialysis program

Education:
Minimum –
- If Nurse Practitioner: Master’s or Doctorate’s degree from accredited Nursing Program.
- If Physician Assistant: Bachelor’s or Master’s degree from accredited PA Program.
Licenses and Certifications:
Minimum – State of Wisconsin NP or PA licensure
If Nurse Practitioner*:
- Active RN and APRN certifications and licenses, including prescriptive authority, in state(s) of practice AND
- Active board certification as a NP in area of clinical practice. Adult; Adult/Gerontology Primary Care, Adult/Gerontology Acute Care.
All Nurse Practitioner and Physician Assistant applicants must have*: Active DEA license or ability to apply for such license prior to or upon hire.
