Description

Job Title: Registered Nurse - Home Care
Location Coverage: Lancaster County, PA
Duration: 5 Months
Shift Timings: Day time. 8 am - 5 pm with 1 hr for lunch (hours flexible) , weekly meeting 1 hr, every 4th weekend
NOTE: Need to use their own vehicle

Required: 2 years RN-Home Care experience with OASIS documentation.
Job Description:

Conduct initial patient assessments to develop individualized care plans tailored to the patient's needs and preferences.
Provide skilled nursing care, including wound care, medication management, and symptom management, in the comfort of patients' homes.
Educate patients and their families on disease management, medication administration, and other aspects of care to promote independence and improve outcomes.
Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to coordinate comprehensive care for patients in the home setting.
Monitor patients' health status and response to treatment, adjusting care plans as needed to optimize outcomes and ensure patient safety.
Document all care provided accurately and timely, maintaining thorough and up-to-date patient records in compliance with regulatory standards and agency policies.
