Job Summary:
The Nurse Navigator provides procedural coordination to the Cardiology Program to include, but not limited to: Outpatient ED Testing Referrals to Cardiology, Obtaining Insurance Pre-Authorization, Cardiac Monitor Program, Lipid Clinic, Watchman, TAVR, and Mitra Clip. The nurse works collaboratively with the Cardiovascular Administrative Director, Regional Director of Cardiology Operations, physicians from multi-specialties in developing referrals, coordinating clinic and outpatient visits to navigate clinical care accurately and effectively. The Program Navigator is responsible for organizing physician-team meetings, compiling clinical data for presentation, and following up on team recommendations. The Program Navigator provides clinical education and support to other nursing departments in respect to procedures and testing. The Nurse Navigator monitors and assures that patient care is in compliance with CMS required clinical registries associated with any high-tech procedures.
Qualifications:
- Skilled in critical care nursing and operating room or catheterization lab experience.
- Basic cardiovascular rhythm recognition and cardiopulmonary monitoring assessment.
- Advanced knowledge of cardiovascular diagnostics and imaging techniques.
- Excellent interpersonal and communication skills, written and verbal.
- Ability to problem solve and multitask.
- Highly organized in compiling clinical data / images for presentation to multi-specialty physician teams to accurately assess patients for advanced clinical procedures.
- Diplomatic personality with strong drive to grow the program.
- Registered Nurse with a minimum 5 years of experience cardiac critical care.
