We are seeking compassionate and skilled Registered Nurses to join our dynamic Hospital at Home team in McKinney, Texas. As a Hospital at Home Registered Nurse, you will have the unique opportunity to provide personalized care to patients within their homes. You will play a crucial role in assessing clinical and environmental needs, fostering a healing environment, and supporting patients and their families through education and emotional support.
KEY RESPONSIBILITIES
· Provide high-quality acute care services to patients enrolled in the Hospital at Home program.
· Conduct comprehensive assessments and develop individualized care plans tailored to each patient's needs.
· Spend additional time at bedside assessing clinical and environmental needs and supporting an overall healing environment.
· Act as a key educator and provide ongoing support to patients and their families, ensuring they have the necessary resources and understanding to manage their care at home.
· Collaborate closely with interdisciplinary teams to coordinate care and optimize patient outcomes.
QUALIFICATIONS
EDUCATION - Grad of an Accredited Program
MAJOR - Nursing
CERTIFICATION/LICENSE/REGISTRATION - Registered Nurse (RN)
Basic Life Support (BLS): BLS or BLS within 30 days of hire or transfer.
EXPERIENCE – 1 year of Nursing experience required, 2 years of Nursing experience strongly preferred.
Recent hands-on Acute and/or Critical Care experience required.
Strongly prefer nursing experience that has required independent work relevant to this role- Caring for higher acuity patients, managing their resources, vitals, medication management, IVs, blood draws, dressing changes, and collaborating with physicians, lab, etc.
