AMDIJP00008402 RN Hospice Description:

Provides coordinates and directs the provision of home nursing care according to physicians orders based on agency policies and procedures through the competent application of the nursing process. Responsible to Clinical Manager Directly Supervises and Evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
DEGREE OF TRAVEL
Home visits. Periodic office meetings. Must have reliable transportation current drivers license and agency-required liability insurance.'
Average # of patients/day: 5-8
Points per day/week: How are points broken down 30 points per week. Each type of visit and activity (documentation travel etc.) hold a different point value. Contractor will be assigned between 5-10 patients per day that equate to roughly 6-8 points.
Guaranteed Hours: No if there is a situation where the contractor is unable to meet their financial needs due to low patient census please reach out to the AM to discuss floating the contractor to a nearby clinic to pick up hours.
Types of Cases: Start of Care Wound Care Follow up Visits etc.
Years of experience REQ: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience depending on the specialty
First-timers accepted: (Y/N) No must have recent specialty experience.
Weekend REQ: Varies per location and profession please have traveler discuss during the interview.
Certs REQ: CPR BLS

Territory/County covering: Territory mileage radius Must be willing to travel up to 75 miles from home clinic.
Mileage reimbursed IRS Standard rate. Traveler is compensated from patient home to patient home. The commute to and from the first and last patient are not billable.

Some positions specifically in the western territory often have a Baylor Shift schedule: 4x10s Friday-Monday. Most will be standard 5x8s M-F.

Dress Code: Scrubs
Charting: All professions must be proficient with HomeCare HomeBase (HCHB) and used it within the last 6 months. If they have used it outside of the last 6 months they may apply but will be required to take a retraining course during compliance. RNs PTs and Ots are required to be proficient in Oasis. How long is the contract assignment 8 weeks -Schedule of assignment hours Monday- Friday- 8 a.m. 5 p.m. -Will there be any weekend rotation No (only back up on call weekend rotation) -Will there be any on call Yes- back up on call- about 1-2 times a week and rotating weekend with the other RN case managers at this time it would be every 4 weekends. This is back up at this time not primary on call. - What region are you in Akron territory- this is from Wayne county to Trumbull county. We are looking to have the support in our growth areas as needed which right now is Cuyahoga Summit Stark Portage Mahoning Trumbull counties. -Which location will this person report to for their assignment: Akron- Compassionate Care Hospice 61 N. Cleveland- Massillon Rd. Suite E Akron OH 44333 -About how many miles per week will the contract travel The goal will be for this person to be in one territory each day so not traveling long distances each day. Approximately about 50-100 per week. -What are the weekly units of productivity expectations- 22-27 per week. -What EMR systems will the consultant be using: Tablet- Point Care- HCHB -Do you want the candidate to have experience in home health Its helpful but not mandatory.

Gaudy and Gina are awesome!!!

I worked a while ago for Tact. I have no idea why there are bad reviews but they are amazing!!! Gaudy deserves such a great recognition for her work and placing me in the epicenter of the pandemic and doing her job amazingly! She was also promoted. Gina recruiter is so nice, good , caring and understanding. I love them the company I just wasn’t able to do a travel job again at the time they were helping me again in 2022/3 but they are great to work for and with!
