Description
Requirements:
- Active, unrestricted Illinois RN, LCSW, or LCPC license required.
- Minimum 3 years of clinical practice experience.
- Case management experience preferred.
- Healthcare or managed care experience preferred.
- Must reside in or near the target counties.
- Willing to travel 50–75% to members’ homes. Mileage reimbursement provided
- Strong assessment, communication, documentation, and organizational skills.
