- 1+ year current ULT/Sonographer Vascular experience Required
- ARDMS/RVT Required
- Cerner experience Preferred
- Cover page (if available/not required)
- Work History
- Skills Checklist (current to 1 year)
- 1 Supervisory Signed Reference from within the past two years of employment
- License/Certifications - Must be uploaded with profile (BLS, ARDMS and PERM ADDRESS)
- ARDMS
- BLS (AHA)
