Description
Start Date: April 20, 2026
Location: Charlotte, North Carolina
Compensation: Details provided to qualified applicants
Schedule: 10-hour day shifts, 8:00 AM – 6:30 PM; 40 hours per week available; 13-week contract; no weekends, no call, no float requirement
Highlights:
- Work in a Skilled Nursing Facility within a hospital setting, providing short-term rehab care for residents with 14-21 day stays
- Patient ratio of 1:12 with an average daily census of 9 in a 12-bed unit
- Hands-on MDS coordination role including completion of assessments, care plan development, CAA completion, and utilization review
- Collaborate with interdisciplinary team including PT/OT, speech therapy, social services, pharmacy, and respiratory services
- Utilize Epic EMR system with 24 hours of unit orientation following initial hospital orientation
- Diverse patient population with diagnoses ranging from generalized weakness to surgical aftercare, wound care, and pressure ulcers
Requirements:
- Bachelor of Science in Nursing (BSN) required
- Minimum 3 years of nursing experience required
- Current MDS experience within the last 3 years required – must have hands-on assessment completion experience, not management-only role
- North Carolina RN license or Compact license required at time of start
- BLS certification required
- Epic charting experience required
- First-time travelers accepted
- Experience in Long Term Acute Care/Skilled Nursing, Managed Care, MDS Coordination, and Rehab settings required
- Proficiency in MDS regulatory requirements, ICD-10 coding, CPT coding and billing, CMS guidelines, and clinical documentation improvement
- Experience with admission criteria, care coordination, discharge planning, utilization review, prior authorizations, and quality improvement processes
- Flu vaccine required (medical/religious exemptions only)
- RAC-CT and ICD-10 certifications preferred
- LTAC experience preferred
