Physical Therapist

Job Details:

• Assess patient conditions and develop individualized plans of care to help clients reach their goals and maximize mobility and independence
• Collaborate with interdisciplinary team and colleagues to ensure patient needs are met
• Perform standard therapy measurement techniques including range of motion, strength, and balance tests
• Utilize a variety of therapeutic modalities to maximize care provided to clients
• Provide personalized, one-on-one care to clients in their own homes
• Perform case management responsibilities
• Vermont PT licensure required
• Home health experience and OASIS experience required
• McKesson/Netsmart/Homecare Homebase EMR experience strongly preferred
• No requested time off - quick start position
