RN Case Manager
StartDate: 7/13/2026 Pay Rate: $1800.00 - $2700.00
POSITION SUMMARY – RN Case Manager
POSITION DUTIES – Collaborates with the interdisciplinary healthcare team efforts to meet facility resource management expectations, assist in the reduction of physician practice variations and enhance patient outcomes with emphasis on decreasing length of stay and monitoring cost effective patient care across the continuum of care. Responsibilities include but are not limited to: determine the medical necessity of all admissions and the need for continued stay, identify the provision of inappropriate or untimely services, assess discharge planning needs for patients of all ages and coordinate the delivery of services to meet those needs, facilitate timely and appropriate discharges from the hospital, identify opportunities for improvement in patient care, and provide documentation of all functions as required by regulatory agencies
MINIMUM REQUIRED QUALIFICATIONS –
- FL/compact RN license
- BLS
- 2+ years minimum of acute care hospital experience as a case manager
- Ability to independently complete a medical necessity review for Medicare and other non-commercial payers, and for patients in Observation, irrespective of the payer source.
- Ability to independently complete a 3008, PASRR, and complete referrals to SNF/HHC/ARF.
- Knowledge of Medicare conditions of participation including the 2 midnight rule and the 3 day stay requirement for SNF discharges.
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – Days- 4 10s. Core hours of 8a-4p. Sat-Tues shift
