Job Summary

Seeking an experienced Hospital Pharmacist for a 20-week contract in Glenwood Springs, Colorado. This role features rotating day/evening shifts with weekend coverage and focuses on medication dispensing, clinical pharmacy services, and supporting an EMR transition from Meditech to Epic.

Responsibilities

• Interpret and verify medication orders and maintain accurate patient medication profiles
• Prepare and dispense medications, including IV admixtures, chemotherapy, and parenteral nutrition
• Monitor drug therapies for interactions, contraindications, and appropriate dosing
• Perform pharmacokinetic dosing per established protocols
• Review culture and sensitivity data to recommend appropriate antibiotic therapy
• Provide clinical consultation and drug information to providers, patients, and families
• Document clinical interventions and ensure compliance with all regulatory requirements
• Participate in quality improvement initiatives and medication safety programs

Required Experience

• Minimum 3 years of hospital pharmacist experience
• Strong experience with Epic (required)
• Experience with Meditech EMR preferred

Required Education

• Graduate of an ACPE-accredited School of Pharmacy (BS Pharmacy or higher)

Required Certifications

• Active Colorado Pharmacist License
• BLS (American Heart Association)
• ACLS (American Heart Association)
