- Provide direct patient care for medically complex, long-stay patients
- Monitor vital signs and patient status, identifying changes and intervening as needed
- Administer medications and IV therapies (including antibiotics, TPN, drips)
- Perform wound care, trach care, and ventilator management
- Develop, implement, and update individualized care plans [interviewguy.com]
- Collaborate with physicians, respiratory therapy, PT/OT, and case management
- Educate patients and families on conditions, recovery, and discharge planning
- Accurately document care, assessments, and outcomes
