This position functions as a RN Case Manager for the Case Management Department under the direction of the Director of Case Management. * Incorporates the fundamental principles of utilization management, monitoring resource consumption, social services intervention and individual discharge planning that will include assuring that the transfer or discharge of a patient to another level of care, treatment, services or different setting is always based on the patient's assessed needs and the organizations capabilities to meet these needs. * Facilitates delivery of clinical and community services to patients and families through effective utilization of available resources. * Manages the observation patients on their assigned unit, conduct InterQual reviews for admission, and continued stay per policy, report avoidable days and facilitate clinical reviews to third party payors * Ensures the appropriateness and cost effectiveness of patient's plan of care. Report all complex cases to the Director and/or Care Coordination Manager as often as necessary. * Proactively collaborates with physicians(s) to develop patient care plans and review for medical necessity for continued hospital services and resource consumption. MINIMUM QUALIFICATIONS: * BSN graduate of an accredited school of nursing. * Valid State of Florida RN license. * Minimum of one (1) year in Hospital Case Management/nursing. * Working knowledge of patient assessment, medical terminology. * Current BLS - CPR Certification.
