Registered Nurse Healthcare Coordinator

We are seeking an experienced Registered Nurse (Healthcare Coordinator) to work in our beautiful state of the art facility. RELOCATION TO BEAUTIFUL GILLETTE, WYOMING IS REQUIRED. The ideal candidate for the Registered Nurse (Health Coordinator) position will be responsible for improving patient experience. Salary range $80,000 to $90,000.

Registered Nurse (Healthcare Coordinator) Qualifications and Responsibilities:

• Active Registered Nurse (RN) or Practical Nurse (LPN) license in Wyoming, or be eligible 

• Relevant certifications (case management, care coordination, patient navigation) are a plus

• 2+ years of clinical experience in a hospital, outpatient clinic, or ambulatory care setting

• Experience in care coordination/case management/population health or preventive care

• Prior experience using EMR systems regularly, and pulling reports

• Coordinate and schedule appointments/services, following up to ensure care is completed

• Document all interactions and referrals in EMR; maintain accurate records for continuity of care

Registered Nurse (Healthcare Coordinator) Compensation & Benefits:
