Case Manager RN

Job Summary * The Case Manager is responsible to assist in the development, planning, coordination and administration of the activities of Utilization Review. * Conducts concurrent review of the medical record to determine appropriateness and medical necessity of admission, continued hospital stay, and use of ancillary services. * Assumes a leadership role within the interdisciplinary team to coordinate services through the continuum of care to achieve optimal clinical and resource outcomes. * The Case Manager will work with the insurance companies as well as work directly with patients to arrange discharge,etc. * The Case Manager will work on Med/Surg units as well as ICU, ER and in-patient psych unit- rotating between units * The work schedule is Monday - Friday 7:30am - 4:00pm, as well as every 7th weekend (both Saturday and Sunday), and 1 holiday per year #POTTS Qualifications Education Requirements * Graduate of an accredited school of Nursing Experience * Case Management experience Certification and Licensure * PA Registered Nurse License Required Skills * Analytical Skills * Collaborative Skills * Detail Oriented * Excellent Communications Skills * Excellent Interpersonal Skills * General Clerical Skills * Good Reasoning Skills * Medical Terminology * Multitasking * Organizational Skills * Prepare & Give Presentations * Problem Solving Skills * Strategic Thinking * Strong Team Player Overview Tower Health, a regional healthcare system, serves communities across multiple counties in Pennsylvania.
