Licensed Practical Nurse (LPN) - Home Health

Benefits: Competitive pay range: $79,000-$88,000/ year (negotiable based on experience) Full-time employees receive Medical, Dental, and Vision benefits Mileage reimbursement provided Opportunity for career growth and development Qualifications: Graduate of an accredited practical nurse or vocational nursing program Minimum of one (1) year of experience, preferably in home care or med/surg Current licensure in state and CPR/BLS certificate Excellent communication and problem-solving skills Ability to meet physical demands of the job, including lifting and standing Duties: Implement care plans initiated by the clinician Provide accurate and timely documentation consistent with the plan of care Assess and educate patients and their families/caregivers Coordinate home health services and communicate needs to other disciplines (HHA, OT, PT, MSW, ST, Dietitian) Join us at Healthy Living at Home and bring compassionate care directly to our community's homes.
